My Recovery Plan

My Recovery Plan

MRP is an Electronic Health Record Software for clients meant to assess, measure outcomes, and record the journey while building Recovery Capital to overcome addiction.

Developed by David Best Ph.D., the My Recovery Plan tool is an evidence-based platform that provides a finite, measurable recovery plan coupled with an accurate and predictive assessment. It is recovery path neutral and appropriate for delivery in clinical and non-clinical environments by both certified peers and medical professionals.

My Recovery Plan Software

• Measures Recovery Capital at all stages

• Measurable Plans, Mentor Clients, Monitors Progress and/or Challenges

• Measures Recovery Capital and history

• Clinicians and patients have unique access and user profiles

• Clients create their own Recovery Plan with/ without the help of clinicians

Homes 4 Hope utilizes the My Recovery Plan software throughout the clients stay and is our main tool for monitoring continued sobriety after a client has moved out of a H4H facility. With My Recovery Plan, our Counsellors, Recovery Coaches and other facility members plan and create celebrations for 3-month, 6-month and 1-year sober living anniversaries along with other milestones. As a gesture towards our perpetual relationship with our clients, we use the MRP to measure positive achievements after months of leaving the facility and grant long term opportunities including, but not limited to: Certificate celebrations, keynote speakers, the opportunity to become a facility Recovery Coach and more!

12 Steps of Recovery

Recovery Based Programs

Recovery based programs can be support groups for people battling a variety of destructive behaviors, including substance use disorders.  These types of programs encourage members and has helped many achieve and maintain abstinence from behavioral problems such as substance use disorders, gambling addiction and eating disorders. The bonds formed and lessons learned during these meetings can last a lifetime.

Attendance to any accredited recovery-based program is recommended a minimum of 2 times/week for Homes 4 Hope Sober living facility clients. Homes 4 Hope Pre-Treatment clients are required to attend recovery based programs a minimum of 5 times/week. If all Homes 4 Hope Sober Living Facility household clients attend 2 meetings/week for a month straight, a rebate/incentive is given to the household.

Here 2 Help

Here 2 Help Accountability System

Recovery is not a linear journey, particularly when moving into a new environment with questionable stressors as well as other unknown variables. The Here 2 Help Accountability System was created to support client’s smooth transition into a sober living facility that is upfront, straight forward, and comfortable.

The Here 2 Help Handbook solidifies the rules and responsibilities always expected of clients whether in or outside of the sober living facility keeping everyone accountable for their part in the dwelling as well as in society.  With lots of support flowing from house mates, Recovery Coach, Regional Manager etc., in this environment our clients know they’re not alone.

  2. Individual counselling - Each client is afforded a minimum of one individual counselling session per week giving them the freedom to express themselves in private.

  3. Daily activities - This includes: chores, lectures, workshops, health and fitness, as well as community projects
